The agent
An AI agent for health records that can show its rules.
It answers questions about one patient's documents with citations, under a written policy of 23 rules. Each rule comes from HIPAA, another privacy law, or one of Arbiter's own measured failures. It runs on a local model, sees only de-identified text, and records on every audit row which policy version governed the answer.
What it can do
- Search one patient's records and read that patient's structured facts.
- Answer with a citation to the exact excerpt, or hand the request to a person.
- Write for a clinician or, with a state named, for the patient, with the disclosures that state requires.
What it can never do
- Send identifiable text to a model, or call a model that is not local or under a BAA.
- Read beyond one patient, or read psychotherapy notes or substance-use treatment records without the consent the law requires.
- Make a coverage or medical-necessity decision.
- Deliver an answer that failed a rule or the grounding check.
- Write a question, excerpt or answer into the audit log.
Where the rules run
- When the worker starts: 7 rules
- When a request arrives: 3 rules
- On every tool call: 2 rules
- On the excerpts a tool returns: 2 rules
- On the draft answer: 4 rules
- Before the answer leaves: 2 rules
- Before a release counts as verified: 3 rules
How the rules change
Every two to three days the research routine reads new primary sources and Arbiter's own results. When a finding should change the agent's behavior, it writes a proposal with the evidence and a test on its own branch. A person reviews and merges it, and the change appears in the policy log. The routine never merges its own changes.
Latest scenario run
Each scenario states the outcome the policy requires; the run fails if any misses it. Policy 1.0.0+c4e1be81cbd8, synthetic records, qwen2.5 7B served locally.

What code cannot supply
The policy encodes technical safeguards. A deployment that handles real patient data also needs:
- Signed business associate agreements with every tenant and every vendor that stores, processes or transmits the data, and hosting under a BAA.
- A written risk analysis, policies and procedures, a security officer, workforce training and a sanctions policy.
- Consents taken from the organization's own records, not typed by the caller.
- TLS everywhere, managed encryption keys, write-once audit storage kept six years, and tested backups.
- A clinician workflow for escalations, and de-identification measured on real annotated records.
